Duties & ResponsibilitiesDirect Patient Care & Clinical Interventions
  • Medication Administration: Safely administer prescribed medications and treatments in strict accordance with healthcare provider orders and safety protocols.

  • Clinical Assessments: Perform focused patient health screenings and assessments, accurately documenting physical and behavioral findings in real time.

  • Routine Nursing Care: Deliver high-quality wound care, assist with chronic disease management, and execute routine nursing interventions to support patient wellness.

  • Intake & Discharge Coordination: Assist with critical patient intake screenings, initial health history reviews, and comprehensive discharge planning to ensure a smooth transition of care.

  • Health Status Tracking: Monitor and evaluate ongoing patient health status, remaining highly vigilant to subtle changes and promptly reporting significant updates to the RN or provider.

  • Emergency Response: Respond swiftly, calmly, and appropriately to medical emergencies or urgent healthcare needs within the facility.

  • Patient Education: Empower patients by educating them clearly on their chronic health conditions, medication compliance, and prescribed treatment plans.

  • Team Synergy: Collaborate seamlessly with the broader healthcare team and facility staff to ensure continuous, uninterrupted care.

  • Meticulous Documentation: Maintain clear, concise, and regulatory-compliant medical records following established facility guidelines.

  • Safety & Infection Control: Strictly adhere to universal precautions, infection control procedures, and all facility safety protocols at all times.

  • Other duties as assigned.

Required Qualifications
  • Licensure: Current, active, and unrestricted Licensed Practical Nurse (LPN) license.

  • Certifications: Current Basic Life Support (BLS) / CPR certification.

  • Clinical Experience: Minimum of one (1) year of professional nursing experience.
