Job description

Department: Case Management

Shifts Available:  Days 

Employment Type: Per diem / PRN 

Hours: 8-hour shifts – 8:00 am to 4:30 pm

Location: Providence Medical Center- Kansas City, KS

The Case Manager LPN is responsible for coordinating patient care, discharge planning, and utilization review to ensure patients receive appropriate, cost-effective care across the continuum. This role collaborates with physicians, nursing staff, and interdisciplinary teams to optimize patient outcomes, reduce length of stay, and ensure compliance with regulatory and payer requirements.

Essential Duties and Responsibilities (includes, but is not limited to): 

  • Coordinate and manage individualized patient care plans from admission through discharge
  • Perform ongoing patient assessments and monitor progress toward goals of care
  • Facilitate discharge planning, including post-acute placement, home health, DME, and community resources
  • Collaborate with physicians, nursing staff, social workers, and ancillary departments to ensure continuity of care
  • Educate patients and families on treatment plans, discharge instructions, and available resources
  • Participate in interdisciplinary rounds to proactively address barriers to discharge

Required qualifications:

  • BSW or other bachelor's degree in a clinical healthcare-related field.
  • At least one year of experience in case management, discharge planning, or nursing management
  •  LVN/LPN graduate from an accredited school of nursing, with a current LVN/LPN license, and 2 years of acute care experience.
  • Experience and knowledge in basic to intermediate computer skills.

Preferred qualifications:

  • Minimum 5 years of acute care experience preferred.
  • Case management certification within 2 years of hiring.
  • At least one year of experience in case management, discharge planning, or management is preferred.
  • Current BLS certificate preferred.
  • Knowledge of Milliman Criteria and InterQual Criteria preferred.
