Licensed Practical Nurse LPN
Job Summary
Licensed Practical Nurse Seeking a Licensed Practical Nurse/LPN who will be responsible for providing direct patient care . Will be responsible for working collaboratively with the members of the healthcare team to help meet positive patient care outcomes.
Job Type:
Responsibilities and Duties
Responsibilities of Licensed Practical Nurse/LPN include but not limited to the following:
  • Implement plan of care initiated by the clinician
  • Provides accurate and timely documentation
  • Assess and provides patient and family caregiver education and information pertinent to diagnosis and plan of care
  • Participates in coordination of home health services to the clinician and/or clinical supervisor

Qualifications and skills:
  • One year med surg experience
  • One year Home Health experience required.
  • Louisiana State Board of LPN license required
