Position Overview

Effective Change is hiring Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide in-home nursing care for an individual in Lebanon County, PA.

This is a hands-on nursing role requiring:

  • excellent clinical judgment,

  • professionalism,

  • reliability, and

  • a strong commitment to supporting individuals in a home-based setting.

We offer both weekend 24-hour coverage opportunities and flexible weekday scheduling, allowing nurses to choose the shifts that best fit their availability.

Responsibilities

  • Provide direct nursing care according to the individualโ€™s care plan.

  • Administer medications and treatments safely and accurately.

  • Monitor, document, and report changes in health status.

  • Maintain accurate and timely documentation.

  • Collaborate with the Effective Change team, family, and other professionals to ensure quality care.

  • Uphold safety, infection control, and nursing standards.

Qualifications

  • Current Pennsylvania RN or LPN license in good standing.

  • Experience in home care, long-term care, or a related setting preferred.

  • Strong communication and interpersonal skills.
