Job description

Job Title: LPN Staff III
Department: Victor Family Medicine
Location: 140 Cobblestone Court Drive Victor, NY 14564
Hours Per Week: 40
Schedule: Full-time Days
SUMMARY:
The LPN III is responsible for providing direct patient care, usually under the direction of a Registered Nurse or Provider, in accordance with established plans. The LPN maintains quality patient care through consistent standards of nursing practice in collaboration with the multidisciplinary team. The LPN is instrumental in providing an exceptional experience for patient and family.
RESPONSIBILITIES:
  • Conducts and documents a thorough observation of each patient's medical status upon presentation and throughout the patient's course of treatment
  • Assists in the implementation of the patients treatment plan under the direction of an RN or provider
  • Provides routine nursing services for patients as directed by RN or provider
  • Complies with the evaluation, treatment and documentation requirements of the department
  • Completes required documentation in an accurate and timely manner
  • Provides regular patient status updates to appropriate personnel
  • Attends and participates in department/facility meetings as required
  • Acts in compliance with hospital, regulatory and professional standards and policies
  • Maintains clinical competency by pursuing continuing education opportunities
  • Performs other duties as assigned

REQUIRED QUALIFICATIONS:
  • Graduate from an accredited School of Practical Nursing
    OR ASN/BSN in Nursing
  • Current New York State Licensed Practical Nurse license
  • Acute Care and Dialysis Only - BLS

PREFERRED QUALIFICATIONS:
  • 1 year of experience preferred.

EDUCATION:
LICENSES / CERTIFICATIONS:
BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
