Job description

Supervised by a Registered Nurse, Physician, or licensed Advanced Practice Provider (APP), the LPN provides guides patients through the ambulatory visit process and ensures care is of the highest quality, delivered in the most efficient manner. The LPN is part of the clinical team maintaining the flow of the provider schedules during patient care sessions, minimizing patient wait time whenever possible. The LPN provides assistance to providers for patient care follow up needs.
The LPN may participate in phone calls and/or in person visits with patients to document symptoms, questions, or concerns. This information will be relayed to the RN or provider for the determination of any action needed. The triage of care situations is the responsibility of the RN or provider; scheduling of acute care appointments can be facilitated by the LPN.
Essential Job Functions: Practices within the Licensed Practical Nurse scope of practice as described in the New York State Nurse Practice Act. The primary job duties of this position are:

$25.79-$33.20/hourly
