Job description

PLEASE READ:

AID Residential Services is seeking a LPN Skilled Nurse Manager to join our team. In this role, you would work a flexible schedule ensuring the health and well being of individuals with intellectual and developmental disabilities, while providing essential updates to their families and care teams. To do this you will lead and manage our Medical Assistants and focus on training and educating staff, including teaching our Core classes, to ensure they are able to provide the highest level of care and support.

Our LPN Skilled Nurse Managers work both in-office and in client homes if necessary, working a flexible schedule. During weekend rotations, you’ll be responsible for phone-based support and responding to any emergent needs, though on-site presence is generally not required.

LPN Skilled Nurse Manager ROLE AND RESPONSIBILITIES

  • Ensure individual and/or staff are appropriately trained and educated on property use and upkeep of all ordered adaptive and durable medical equipment.
  • Work with other LPNs to ensure the health and wellbeing of individuals with I/DD.
  • Complete all physical assessments in accordance with Wellness Coordination guidelines and as needed (ie. post hospital or rehab facility discharge, after a head injury, fracture, etc.)
  • Sends follow up emails to the individual support team after NP appointments.
  • Ensure all required documentation is available and organized for practitioner review and signature.
  • Attend Quarterlies/Annuals; Ensure the individuals support team has the quarterly/annual healthcare report prior to the meeting, scans into the Drive, sends via email to the DON to upload the report into the BDDS Portal.
  • Adds, assigns, uploads, and monitors training in Relias as needed.
  • Trains new staff on QuickMar, and adds all pertinent information in QuickMar for individuals.
  • Risk Plan Management; directly supervises the implementation, coordination, maintenance, evaluation of risk plans and services. Analyzes outcomes. Responsible for updating annually and sending to the teams of the clients and uploading into the DDRS Portal, the Shared Drive and Therap.
  • Oversees initial start of care and assessment. Entering all Medical History and Medication Administration in Therap. Data Entry of; Medications, Dx, Hx of Dx, Allergies/Allergic Reactions, Physicians and Specialists Information, Medication Hx List.
  • Follow up on GERs if needed and advise DON with information needed for IR’s.
  • Teach Core A and B.
  • Complete all annual TB skin tests for individuals caseload and employees when indicated.
  • Admit new individuals with Wellness Coordination; attend all individualized support training including BSP, ISP, and risk plan training. Trains on individual risk plans. Attends day of transition to account for and organize all medications, completes admitting physical assessment, coordinates with individual and programming to schedule follow up appointments as needed.
  • Fields phone calls during the week and rotating weekends for individual triage.
  • Attends all assigned meetings including but limited to weekly SWOT meetings, Interdepartmental meetings, weekly meetings with PM’s, etc.
  • Follow up documentation for all training, physical assessments, and phone calls.
  • Must be able to lift 50 lbs.
  • Must be able to work in houses when needed.
  • Demonstrates the Culture and Values of AID of Indiana.
  • All other duties assigned.
  • Requires LPN certification.

QUALIFICATIONS AND EDUCATION REQUIREMENTS: Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN), BSN preferred; 2+ years managerial experience; Experience with ID/DD population preferred; Proof of valid Indiana nursing license; Valid Driver’s License; Current Auto Insurance.
