Klarus Home Health is looking for full-time, part-time or PRN Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the This position will primarily cover the South Valley, Valencia County and eastern side of Albuquerque.
The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
Requirements
Key Responsibilities:
  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary group, patient/family, and referring agency.
  • Assist in the development and implementation of patient care plans.
  • Monitor patient health and report any changes to the supervising RN.
  • Administer medications and treatments as prescribed.
  • Educate patients and their families on health care needs, conditions, and options.
  • Maintain accurate and timely documentation of patient care and progress.

Job Qualifications:
  • Education: Graduate of an accredited School of Vocational/Practical Nursing.
  • Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
  • Experience: Two years' experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.
  • EMR system: HCHB experience highly preferred
  • Skills: Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
  • Transportation: Reliable transportation and valid, current auto liability insurance.

  • Work in patients' homes in various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
  • Ability to work a flexible schedule and travel locally.
  • Some exposure to unpleasant weather conditions.
