The LPN Intake Nurse - Home Health contributes to our Mission of Caring by receiving and reviewing incoming referrals for home health services, facilitates timely and accurate onboarding, and ensures efficient coordination among field-based clinical staff. This role requires strong clinical expertise, attention to detail, and effective professional communication with clinical staff and internal and external stakeholders.
Qualifications for a LPN Intake Nurse - Home Health:
  • Unencumbered, active LPN license issued by the RI DOH.
  • At least 3 years of experience in a healthcare or home health care setting, with a focus on patient intake or coordination is preferred.
  • Ability to work independently with minimal supervision.
  • Possess high attention to detail and excellent interpersonal and organizational skills.
  • Knowledge of healthcare regulations and insurance guidelines.
  • Strong knowledge of medical terminology, disease processes, and treatment options.
  • Excellent analytical, critical thinking, and decision-making skills.
  • Proficient in accessing and interpreting electronic health records.
