LPN Intake Nurse

Full Time | Monday through Friday from 8:00 am - 4:30pm | No Weekends!

*In Office in Warwick, RI*


The LPN Intake Nurse contributes to our Mission of Caring by receiving and reviewing incoming referrals for home health services, facilitates timely and accurate onboarding, and ensures efficient coordination among field-based clinical staff. This role requires strong clinical expertise, attention to detail, and effective professional communication with clinical staff and internal and external stakeholders.

Qualifications for an LPN Intake Nurse:

  • Unencumbered, active LPN license issued by the RI DOH.
  • At least 3 years of experience in a healthcare or home health care setting, with a focus on patient intake or coordination is preferred.
  • Ability to work independently with minimal supervision.
  • Possess high attention to detail and excellent interpersonal and organizational skills.
  • Knowledge of healthcare regulations and insurance guidelines.
  • Strong knowledge of medical terminology, disease processes, and treatment options.
  • Excellent analytical, critical thinking, and decision-making skills.
  • Proficient in accessing and interpreting electronic health records.
