Licensed Practical Nurse (LPN) Floater

Full-Time | Mercer County, WV

Position Summary

Southern Highlands Community Mental Health Center is seeking a compassionate and adaptable Licensed Practical Nurse (LPN) Floater to provide nursing support across multiple programs within Mercer County. This position plays a vital role in promoting integrated, person-centered care by collaborating with medical providers, case managers, therapists, and other members of the treatment team.

The LPN Floater assists with medication administration and delivery, monitors consumer health status, provides health education, and supports continuity of care by coordinating referrals and communication with medical and community providers. This position requires flexibility to meet the needs of various programs while delivering high-quality, compassionate care to the individuals we serve.

Essential Responsibilities
  • Assist with the daily dispensing and delivery of medications.
  • Monitor and document consumer health by obtaining vital signs, weights, and symptom assessments.
  • Recognize and respond appropriately to signs and symptoms of illness or injury.
  • Coordinate referrals and collaborate with medical providers and community resources to support continuity of care.
  • Complete medication counts and maintain accurate medication records.
  • Assist with prescription prior authorizations and other medication-related documentation.
  • Provide health education and wellness support to consumers.
  • Participate in required trainings and maintain compliance with agency policies and procedures.
  • Work collaboratively with multidisciplinary treatment teams to support individualized care plans.
  • Float between programs as needed to meet operational and staffing needs.
QualificationsRequired
  • Current, unrestricted Licensed Practical Nurse (LPN) license in the State of West Virginia.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
Preferred
  • Experience working with individuals receiving behavioral health and/or substance use treatment services.
  • Experience in community mental health or integrated healthcare settings.
Schedule
  • Full-time position.
  • Flexibility to work 8-hour and 12-hour shifts, depending on program and organizational needs.
  • Cross-training across multiple programs is required to support agency operations.
