LPN Case Managers work closely with registered nurses, physicians, social workers, and other healthcare professionals to coordinate patient care. They often serve as a liaison, communicating patient needs and care plans between different departments and providers. This collaborative environment helps ensure continuity of care and improves patient outcomes. Strong teamwork and effective communication skills are essential, as LPN Case Managers may participate in care conferences, update records, and advocate for patient services.

What is an LPN case manager?

LPN Case Managers are Licensed Practical Nurses who coordinate and oversee patient care plans, often working with healthcare teams to ensure patients receive appropriate services. Their responsibilities include assessing patient needs, monitoring progress, communicating with families and providers, and helping arrange for necessary resources or follow-up care. While their scope of practice is more limited than Registered Nurse (RN) case managers, LPN Case Managers play a vital role in supporting patient outcomes and continuity of care, especially in long-term care, home health, or rehabilitation settings.

Can an LPN do case management?

Licensed Practical Nurses (LPNs) can perform case management tasks under the supervision of registered nurses or physicians, including coordinating patient care, monitoring progress, and documenting health information. However, their scope of practice varies by state and employer, and additional certifications or training may be required for advanced case management responsibilities.

To thrive as an LPN Case Manager, you need a practical nursing license, strong patient assessment abilities, and knowledge of care coordination principles.
