Job Overview:

We are currently seeking a compassionate and skilled Home Health

LPN Case Manager to provide personalized, one-on-one patient care throughout Roanoke and the surrounding coverage areas. This position is ideal for an LPN who enjoys working independently while advocating for patients and helping coordinate quality care within the comfort of their homes.

As a Home Health Licensed Practical Nurse โ€“ LPN Case Manager, you will provide home-based nursing care for patients recovering from illness, injury, surgery, or chronic conditions while working closely with RNs, physicians, and interdisciplinary care team members. This role also includes assisting with care coordination, patient follow-up, scheduling needs, and ensuring continuity of care for assigned patients.


Responsibilities of Home Health Licensed Practical Nurse โ€“ LPN Case Manager:
  • Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions under the supervision of an RN
  • Monitor patient condition, obtain vital signs, and perform routine nursing assessments
  • Document patient progress and report changes in condition to RN supervisor
  • Assist with implementing individualized patient care plans
  • Educate patients and families on medications, treatment plans, self-care, and home safety
  • Collaborate with physicians, RNs, CNAs, aides, and interdisciplinary team members to coordinate patient care
  • Assist with discharge planning and ensure continuity of care
  • Coordinate patient visits and communicate scheduling or care needs with the clinical team
  • Help monitor patient goals, follow-up care, and overall case progression
  • Identify patient concerns or barriers to care and communicate appropriately with the care team
  • Maintain accurate and timely clinical documentation

Qualifications of Home Health Licensed Practical Nurse โ€“ LPN Case Manager:
  • Graduate of an accredited practical nursing program
  • Active LPN license in the state of Virginia
  • Minimum of 1 year of LPN experience (home health preferred) or completion of office preceptor program
  • Previous case management or care coordination experience preferred
  • CPR certification and TB skin test required
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, communication, and critical thinking skills
