Our RN will administer skilled nursing care to patients in their place of residence. This is performed in accordance with physician orders and plan of care in conjunction with the Registered Nurse Case Manager and under the direction and supervision of Clinical Team Manager. The registered nurse executes the plan of care as established by the Registered Nurse Case Manager and makes necessary revisions as needed, furnishes those services requiring substantial and specialized nursing skill, provides appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs, participates in in-service programs, and supervises and teaches other nursing personnel.

Minimum Qualifications:

  • New York State Licensed RN.
  • One year of experience as a clinical nurse in a hospital or other direct patient care clinical setting required.
  • Current NYS driver’s license

RESPONSIBILITIES:

  • Provide skilled nursing care in the home, including patient assessments, medication and vital sign reviews, IV infusions, wound care, and patient/family education to support independence.
  • Document observations, treatments, and outcomes accurately while ensuring compliance with medical necessity and visit authorizations.
  • Collaborate with the case manager, physicians, patients, families, and the care team to develop, revise, and implement care and discharge plans.
  • Address patient needs and resolve concerns promptly at the point of care.
  • Meet productivity and documentation requirements in alignment with agency and payer standards.
  • Perform additional duties as assigned.

Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.
