Any Urology experience is a plus. ~~~~~~~~~~~~~~~~~~~~~_ SCHEDULE Coverage Type: Outpatient Schedule Type: Clinic only Call Type: N/A Dates of Need: .5-.7 to cover in clinic for Peds ENT (2.5-3 days a week) and for Peds Uro post op visits (1/2-1 day a week). Shift Times: 8:30am-5:00pm ( 8-830am will be admin time. 8:30-12 will be patient care visits, 12-1 lunch, 1-4:30 patient care visits, 4:30-5 admin time) Lunch 1 hour Provider Min # Shifts Required: see additional details Guaranteed Hours per Day: 8 Hours Worked for OT to apply: 9 per day Rates to be Filled Out on Bid: Clinic only: Regular, Orientation, Overtime, Holiday ~~~~~~~~~~~~~~~~~~~~~_ CREDENTIALING Current CV (must be uploaded to bid) License: Active only DEA Required: Yes Boards: BC or truly BE Skills/Certs: BLS Other: ~~~~~~~~~~~~~~~~~~~~~_ ADDITIONAL DETAILS Wound assessment and suture removal. Flexible endoscopy experience preferred. Evaluation of tonsil, adenoid, and otitis media are most common office diagnosis. Experience with nasal cautery, foreign body removal preferred. Coverage will amount out to about 4 days a week possibly as listed above. Provider will need to be credentialed with the payors which will take some time so targeting a end of August, September 1 start date. Interviews will be with the physician leader who is mainly available on Tuesdays between 1-2pm PST. It will be a teams interview. Please list in the bid presentations if this availability will work for your provider or if another time will need to be accomodated.
