Social Worker / Behavioral Health Clinician - Primary Care
Salary Range: $69,535-$102,877/hr based on experience and qualifications

About the Role
We're looking for a dedicated Social Worker /LMHC to join our team and help us continue delivering the level of care our patients deserve. In this role, the Embedded Therapist participates as an integral member of a multi-disciplinary care team in one or more primary care settings. This position provides clinical services, including screenings and comprehensive assessments to individuals with mental health or substance use disorder problems. The Embedded Therapist provides evidence-based psychotherapy services to patients. This position will also provide treatment planning, discharge planning, and referral to services.
What You'll Do
Psychotherapy: Provides screening and comprehensive assessments to primary care patients within the practice. Based on the clinical formulation and diagnosis will provide psychotherapy services. In collaboration with the patient, develop treatment plans and discharge plans. Provide referrals as indicated. Demonstrates excellence in both internal and external customer service.
Case Management: Provides time limited case management services to patients on active caseload. Coordinates case management with nursing staff as appropriate.
Documentation: Exhibits knowledge and skill in documentation of all services in the electronic health record. Ensures that comprehensive assessments, treatment plans, discharge plans, progress notes and documentation of case management activities are completed in accordance with standards and in a timely manner.
Community Network and Resources: Established and maintains positive working relationships within the community agency service system. Establishes and maintains effective network of community resources for cross-referral of patients. Demonstrates ability to continuously develop both external and internal resources to support needed referrals and coordination of care.
Integration: Promotes integration of behavioral health services into the primary care model through coloration with patient care team and attending primary care provider meetings as appropriate.
What You Bring
LCSW OR LMHC required. Minimum of two years' clinical practice in an ambulatory setting with at-risk patients preferred.
Knowledge of available community resources and the ability to evaluate resources and make appropriate referrals.
Knowledge of health-related financial assistance, including Medicare and Medicaid.
