• Act as a liaison between clients and family members, as well as various outpatient providers, to facilitate comprehensive care.
• Engage with insurance providers to manage pre-certification processes and utilization reviews, ensuring proper coverage for necessary services.
• Participate actively in discharge planning and aftercare initiatives, crafting individualized care plans that support ongoing wellness and recovery.
• Provide therapeutic meal support by modeling healthy relationships with food and helping clients navigate their dietary and emotional needs.
• Deliver empathetic, client-centered care that honors the uniqueness of each individual's circumstances, taking into account their intersecting identities and backgrounds.

Your primary focus will be on older adults, a demographic often facing complex personal and emotional challenges. By understanding the unique factors affecting this population, you will play an integral role in enhancing their quality of life during critical times. This includes offering both emotional support and practical solutions that target their mental health and wellness.

• A Master’s degree in Social Work from an accredited institution is required.
• Must possess a valid Licensed Clinical Social Worker (LCSW) license in the state of Arizona.
• Demonstrable experience working with older adults, showcasing an understanding of their specific needs and challenges.
• Strong clinical and diagnostic skills to assess and treat a variety of mental health conditions.
• Excellent communication and interpersonal skills, enabling effective collaboration with clients, families, and fellow team members.
• The ability to work both independently and as part of a cohesive team, contributing to a collaborative atmosphere.

• Experience in hospice or palliative care settings, with a focus on end-of-life issues, is highly desirable.
• Familiarity with insurance processes and utilization management, specifically in the mental health arena.
• Demonstrated success in developing individualized care plans that address the multifaceted needs of clients.
