Key Responsibilities

• Act as a liaison between clients, their families, and various outpatient providers to ensure seamless service delivery and continuity of care.
• Regularly communicate with insurance companies to navigate pre-certification processes and manage utilization effectively, ensuring that clients receive the necessary services without undue delay.
• Participate actively in discharge planning and aftercare strategies, collaborating with interdisciplinary teams to provide wraparound services that support clients' recovery journeys.
• Provide therapeutic meal support by modeling a healthy relationship with food, which involves guiding clients through food-related challenges sensitively.
• Create a safe and empathetic therapeutic environment tailored to address the diverse identities and experiences of each client, ensuring that all individuals feel valued and affirmed.

Patient Population / Client Focus

Qualifications

• Master's degree in Social Work from an accredited university.

Compensation and Benefits
