In this role, you will work on-site at various assisted living facilities, allowing you to engage with residents in their living environment. You will utilize mobile electronic medical record (EMR) tools for documentation, ensuring compliant and efficient patient care processes.

• Deliver on-site physical therapy services to residents in affiliated assisted living facilities, focusing on orthopedic recovery and rehabilitation.
• Conduct comprehensive evaluations to assess residents’ mobility and functional capabilities, developing tailored, orthopedic-informed treatment plans.
• Collaborate with nursing staff, caregivers, and the multidisciplinary medical team to coordinate care efforts and ensure continuity of service.
• Provide education to residents and their families regarding safe mobility practices, individualized home exercise regimens, and overall wellness strategies.
• Utilize mobile EMR systems for precise documentation of evaluations, progress notes, treatment plans, and compliance with HIPAA regulations.
• Act as a liaison between facilities and the multidisciplinary clinic, facilitating referrals and follow-up care to optimize patient outcomes.

This role specifically caters to older adults residing in assisted living environments, a population often facing orthopedic challenges such as joint pain, mobility issues, and post-surgical rehabilitation needs. Your interventions will play a crucial role in enhancing their functional independence, promoting better health outcomes, and improving their overall quality of life.

• Current licensure as a Physical Therapist in the State of New Jersey (or eligibility for licensure).
• A Bachelor’s Degree in Physical Therapy from an accredited institution.
• At least 2 years of relevant experience as a Physical Therapist in outpatient, geriatrics, or similar settings.
• Demonstrated strong interpersonal and communication skills.
• In-depth clinical understanding of orthopedic conditions commonly affecting the elderly.
• Ability to work independently and effectively in a residential environment.

• Experience working in assisted living facilities (ALFs), skilled nursing facilities (SNFs), or home health care settings.
• Familiarity with interdisciplinary collaboration and a patient-centric approach to rehabilitation and care.
• Previous experience leveraging mobile technology tools for patient care and documentation.

The compensation for this position ranges from $90,000 to $100,000 per year, commensurate with experience.
