Job description

A-Line Staffing is now hiring an RN Case Manager. This will be Full Time.

  • ·Location Requirement (Mandatory): Candidates must live in one of the following counties for consideration : Must Reside in NOVA, Sterling, Reston, Chantilly, Oakton, Herndon
  • Schedule – M-F 8am-5pm
  • Pay Rate- $44.14 an hour paid weekly
  • Benefits – active after 90 days of employment

Key Responsibilities

  • Conduct comprehensive member assessments using clinical tools, data review, and eligibility determination
  • Evaluate benefit plans and available internal/external resources to determine appropriate interventions
  • Develop, implement, and adjust individualized care plans based on clinical needs and ongoing evaluation
  • Coordinate care transitions, including hospital discharge and nursing facility transitions
  • Address medical and social determinants of health (housing, food insecurity, etc.)
  • Collaborate with providers, members, caregivers, and care teams to ensure timely service delivery
  • Apply case management standards, regulatory requirements, and internal policies
  • Maintain accurate documentation in electronic health records
  • Support quality and utilization management processes
  • May mentor new team members once proficient in role

Required Qualifications

  • Active, unrestricted RN license in the State of Virginia
  • BSN required (or equivalent as specified in role requirements)
  • Minimum 3 years of clinical practice experience in case management or home health or discharge planning
