The Assisted Living Life Enrichment Coordinator works with others to promote the psychosocial well-being of all residents and assure that there are opportunities in Assisted Living for meaningful pursuits, daily pleasures, recreation, exercise, friendships, social interaction, growth and learning, community service, fun and enjoyment, personal routines and neighborhood rituals.
  • Performing tasks that require a social worker designee and assisting the household as needed in providing other social services.
  • Promotes the residents' well-being and satisfaction by supporting resident-directed care, guides the interdisciplinary team as it establishes and implements the residentsโ€™ plan of care with the resident and his/her family or responsible party and the household team.
  • Participates in creating a warm, inviting neighborhood physical environment that makes people feel at home.
  • Creates community through maintaining "home" in the physical environment, maintains a sense of welcome through elimination of institutional barriers, enhances relationships in the household through shared activities of living, promotes compassionate services among the team by offering emotional, psychological and spiritual support to all, evaluates and addresses the successes and needs of the household.
  • Facilitates the effective completion of care plans and documentation with regards to psychosocial well-being and social service needs.
  • Coordinate scheduling and leadership of resident care plan meetings.
  • Stays visibly available and responds in the moment to opportunities for spontaneous activity and a lively daily life in the neighborhood.
  • Responsible for creating a calendar of planned events.
  • Facilitates the development of planned activities to support life enhancement.
  • Engage in and utilize the Resident portal.
  • Ensures completion of necessary documents such as power of attorney and living will to direct decisions that may be made on their behalf.
  • Determine what support services residents may need to promote their overall well-being
  • Participates in neighborhood team meetings and contributes to the building of teamwork and the work of a self-led team.
  • Assures that Assisted Living has the needed resources (community links, materials and supplies, information, and skills) to be successful in meeting the Life Enrichment needs of residents and grows everyoneโ€™s ability to bring forth a good daily life. Monitors the activity budget for Assisted Living.
  • Assures that life enrichment practices and services support the values of person-centered, resident-directed practice.
  • Assures that resident rights are honored and met.
  • Participates in meeting the transportation needs of residents in Assisted Living, and occasionally in wider Glenaire community as needed.
  • Welcomes and orients new residents to Assisted Living and helps them integrate into the daily life of the neighborhood.
  • Coordinates with the interdisciplinary team related to resident transitions. Assists with resident move into the household to assure resident needs and organizational objectives are met on move-in and move-out of the household.
Required:
  • Excellent English verbal and written communication skills.
  • Excellent interpersonal skills.
  • Ability to create and implement sourcing strategies for recruitment for a variety of roles.
  • Proactive and independent with the ability to take initiative.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Proficient with or the ability to quickly learn related booking software/programs
  • Proficient with Microsoft Office Suite or related software.
Education and Experience:
  • Bachelorโ€™s degree preferred
  • Social Worker degree preferred
  • Valid Driverโ€™s License
