Job description

This position is in West Pasco County, Florida. Gulfside Healthcare Services is seeking a compassionate Registered Nurse (RN) to provide hospice field care throughout West Pasco. Our Hospice Field RNs support patients and families in private residences, assisted living facilities, and skilled nursing facilities while providing comfort-focused care, education, and support during the end-of-life journey.

The Hospice Field RN manages an assigned patient caseload, develops individualized care plans, collaborates with our interdisciplinary team, and provides guidance to LPNs, CNAs, and other members of the care team.

EDUCATION AND QUALIFICATIONS:

• Active Florida Registered Nurse (RN) license or Compact State RN license required
• Minimum of one (1) year nursing experience preferred
• Hospice, home health, case management, or acute care experience preferred
• Strong assessment, communication, critical thinking, and time management skills
• Compassionate, organized, and able to work independently while supporting a team-based approach to care
• Valid Florida driver’s license, reliable transportation, and proof of automobile insurance required
• Proficiency with Electronic Medical Records (EMR) preferred
