Job description

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking qualified Registered Nurse (RN) interested in joining our Wilson, Residential Group Home Services team at our location Sussex County, Delaware.
Some of the responsibilities include the following.
Coordinate the behavioral and physical health care needs of the program participants.
  • Provide psychiatric nursing services consistent with physician's orders.
  • Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
  • Provides ongoing assessment of participant's mental health symptoms and responds to recovery plans.
  • Educates and supervises participants in self-administration of medication.
  • Act as a liaison to the Certified Nursing Assistant (CNA) and the Resident Manager on medical and activities of daily living (ADL) and instrumental activities of daily
    living ADL.
  • Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.
  • Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
  • Monitors medication, documenting participant compliance, medication efficacy, and side effects as reported.

Qualifications:
  • Board Certified Licensed RN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
  • MA/BA in nursing or a registered nurse with a Mental Health Certification from the American Nursing Association, preferred.
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for participant rights and personal preferences in treatment are essential.
  • Knowledge of mental health and substance abuse systems and supports.
  • Good communication skills both oral and written.
  • Ability to work independently and as part of a focused team.
  • Strong organizational and analytical skills.
  • Possess a valid driver's license with an acceptable driving record.

Scope:
  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.
  • Local travel required, including transporting participants.
  • May be required to report to work during emergencies, including inclement weather.
  • Scheduling flexibility and possible on-call work are necessary.
  • May require crisis intervention in emergency situations.
