Job Summary

We are seeking compassionate and dependable Certified Nursing Assistants (CNAs) / Home Service Workers to provide high-quality, person-centered care to individuals in their homes and community settings, including assisted living and long-term care environments. In this role, you will support clients with daily living activities while promoting dignity, independence, and overall well-being. This position reports to the Homecare Supervisor.

Duties & Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, eating, toileting, and mobility
  • Provide medication reminders in accordance with care plans and HIPAA guidelines
  • Complete accurate daily documentation of services provided
  • Support clients with memory care needs, including dementia and Alzheimerโ€™s care
  • Prepare nutritious meals based on individual dietary needs and assist with feeding when necessary
  • Perform light housekeeping and laundry to maintain a safe and clean living environment
  • Observe, document, and report changes in client condition, behavior, or needs to the Home Care Supervisor
  • Safely operate assistive devices, including Hoyer lifts and transfer equipment
  • Provide emotional support, companionship, and social engagement
  • Communicate effectively with clients and family members
  • Follow safety protocols, including CPR and First Aid procedures
  • Complete incident reports and participate in investigations as required
Requirements:

Requirements

  • Must be at least 18 years of age
  • High school diploma or GED required
  • Successful completion of a state-approved CNA or Personal Care Assistant program, or comparable experience
  • Ability to lift up to 50 pounds
  • Valid CPR and First Aid certification (or willingness to obtain)
  • TB test and physical as required
  • Successful completion of a background check
  • Valid driverโ€™s license
  • Valid work authorization, if applicable
  • Reliable transportation (personal vehicle with insurance or other dependable means)
  • Minimum of 1 year of experience in home and community-based care, assisted living, nursing homes, or hospice care

Preferred Qualifications

  • Experience working with individuals with physical or developmental disabilities, autism, dementia, or Alzheimerโ€™s disease
  • Knowledge of client observation and reporting techniques
  • Familiarity with HIPAA and confidentiality standards
  • Experience using Hoyer lifts and safe transfer techniques
  • Ability to read, write, speak, and understand English as required for the role; bilingual Spanish is a plus
  • Strong dependability and reliability
  • Ability to follow care plans, procedures, and assignments
  • Friendly, caring, and compassionate demeanor
  • Patient-focused mindset with strong communication and teamwork skills
