Responsibilities

  • Advance client's decision-making, productivity and participation in a range of daily activities. Facilitate the client's inclusion in positive community experiences.
  • Promote independence in daily activities yet be mindful of client's intellectual and physical disabilities and assist with meals, cooking, household maintenance, grooming and dressing, as assigned. Maintain current knowledge of client's needs.
  • Implement individual training and support programs under the direction of the case responsible person.
  • Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members offamilies, Regional Center, D.O.R. and other individuals in the community.
  • Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.
  • Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required.
  • Provide positive advocacy when the individual's preferences, needs, or talents are neglected or overlooked.
  • Address challenging behaviors proactively and respectfully, using methods outlined in plans and training.
  • Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs Provides direct service as needed and required.
  • Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.
  • Perform all other job duties as assigned.

Requirements

  • 18 years of age or older.
  • High School Diploma or Equivalent.
  • Aptitude for confidentiality, organizational skills and good interpersonal skills.
  • Certification in an approved First-Aid and CPR class; or successful completion within 45 days of the date of hire.
  • Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.
  • Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.

POSITION REQUIREMENTS
  • Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
  • A Direct Support Professional works as scheduled by the Regional Manager and/or his/her designee. His/her schedule may be either part or full time, depending on the needs of the clients and Adjoin. Overtime may be required as dictated by operational needs, however, no holiday or overtime may be worked without the prior approval of the Direct Support Professional's supervisor.
  • Successful completion of pre-employment drug screen and background check.
  • Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
  • Acceptable DMV record.
