Job description

JOB SUMMARY:

As a Case Manager, you will conduct Long Term Care (LTC) assessments, create and review care plans, and coordinate services with individuals served and their interdisciplinary team in order for them to live, work and go to school in their communities. Connect individuals to resources to support their goals and dreams. Attend meetings on the agency’s behalf, mentor new hires, community outreach, and implement the agency’s racial equity plan. These duties will include culturally specific case management services with the African American, African, Native American, Asian, and Hispanic communities. We have several positions open in Ramsey, Washington, and Hennepin Counties.

REQUIREMENTS:
  • 4year degree in Social Work and Licensed Social Worker by the MN Board of Social Work or related Bachelor’s degree.
  • Must have at least one year of fulltime social service experience in the education or treatment of persons from vulnerable populations
  • Experience in Elderly Waivers, AC, DD, CADI, BI, and CAC strongly preferred..
  • Knowledge of MA, Medicare, and MN Healthcare Programs strongly preferred.
  • Experience and knowledge of working with the following communities: black/AfricanAmerican, East and West African, AmericanIndian, Hmong, Karen, and Latino required
  • Knowledge of historical racial trauma, cultural responsiveness, restorative practices required.
  • Background clearance
  • Ability to effectively communicate in English, written and verbally.
  • Driver's license, reliable vehicle you can use for work purposes, acceptable driving record and proof of auto insurance required.
  • Willing to drive own vehicle to perform job duties.
  • Must be at least 21 years of age.
  • Live in the State of Minnesota.
  • Preferred Skills:

    Experience with SSIS, MnSP (MnChoices Support Plan) System, and person-centered thinking and planning. Strong oral and written communication skills. Time management skills.
