Job description

 Terros Health is pleased to share an exciting and challenging opportunity for a Peer Support Specialist at the Mitchell Health Center.

Pay Starting at $17.00 Per Hour

Under the Agency’s policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness, or General Mental Health diagnosis under Court Ordered Treatment. Assists patients with identifying treatment and recovery goals, and provides supportive services to help patients reach their goals. Provides recovery-based coaching for Patients related to their clinical recovery goals. Assists members in pursuing and achieving their goals through direct one-to-one and group psychosocial support, brief clinical interventions, motivational interviewing, active listening, and sharing their personal shared experience to support the patient’s recovery. Works under the direct supervision of the team Clinical Coordinator.
 

Duties May Include:

  • Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients, their families, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the individualized service plan are implemented.
  • Conduct face-to-face interactions with patients, family, or other involved parties in the home, community, and recovery clinic for the purpose of assessing, maintaining, or enhancing a person’s functioning.
  • Participate in Consumer Advisory Council at the respective site. Promote patient and family/natural support participation in Advisory Council meetings. 
  • Facilitate peer support and psychosocial groups, either in a clinic or community setting, and engage patients in group participation as related to their individual recovery goals.
  • Provide transportation for the patients as appropriate and as medically necessary, and determined by the clinical team, in a personal vehicle or in Terros Health provided vehicle, when available. Coordinate alternative transportation for patients and identified natural supports, as medically necessary.
  • Provide excellent customer service to patients, internal and external provider agencies, state entities, or other stakeholders (e.g., probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
  • Provide direct assistance in finding necessary resources other than covered services to meet basic needs for patients served.
  • Encourage, assist, and support patients in expressing their perspective throughout the processes of assessment, service planning, and service delivery, while maintaining a focus on their recovery.

  • Bilingual pay differential 
  • High School Diploma or GED, and at least 6 weeks of behavioral health experience.
  • The applicant must have self-identified, first-hand experience with family members with serious mental illness and/or a chronic disease, utilize a recovery approach and philosophy, be able to liaison between multiple parties, and exemplify great customer service and motivational skills.
  • Peer Employment Training and Arizona State Peer Certification required.
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.  Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions. Understanding of recovery principles and the ability to utilize personal experience to benefit others. 
  • Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred.
  • Must have a valid Arizona driver’s license, be 21 years of age with a minimum of 4 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming the role.
  • Must pass a TB Test.
