Position Summary

The Transport Coordinator serves as the on-site liaison between assigned healthcare facilities and LifeTrans operations. This position is responsible for coordinating patient transportation requests, resolving scheduling issues in real time, facilitating communication between facility staff and LifeTrans dispatch, and ensuring a high level of customer service throughout the transport process.

The ideal candidate is proactive, highly organized, customer-focused, and able to work independently with minimal supervision. This role requires strong relationship management skills, the ability to solve problems under pressure, and a professional presence within hospital, skilled nursing, rehabilitation, and healthcare environments.

The Transport Coordinator plays a critical role in improving patient flow, discharge coordination, transport efficiency, and customer satisfaction while serving as a direct representative of LifeTrans at the assigned facility.

Essential Duties and Responsibilities

Transport Coordination

• Coordinate and facilitate patient transportation requests between healthcare facility staff and LifeTrans dispatch operations.

• Monitor transport schedules, trip status, delays, and service issues in real time.

• Assist facility personnel with arranging, modifying, escalating, or canceling transportation requests.

• Actively problem solve transport-related issues to minimize delays and service disruptions.

• Maintain awareness of transport priorities, discharge timelines, and operational constraints.

Customer & Facility Relations

• Serve as the primary on-site point of contact for assigned healthcare facilities.

• Build and maintain professional relationships with case managers, discharge planners, nursing staff, social workers, and administrative personnel.

• Maintain a calm, professional, and solution-oriented demeanor during high-pressure situations.

• Advocate for both the customer and operational teams to achieve timely and effective resolutions.

• Support customer retention through responsiveness, professionalism, and follow-through.

Operational Support

• Communicate continuously with dispatch, drivers, supervisors, and facility personnel regarding trip updates and operational changes.

• Assist in identifying service gaps, recurring issues, and operational improvement opportunities.

• Accurately document transport requests, communication notes, service concerns, and resolutions.

• Support implementation of new accounts, operational processes, and workflow improvements.

• Ensure compliance with company policies, HIPAA requirements, EMTALA awareness, and facility procedures where applicable.

Administrative & Reporting Functions

• Track transport activity, delays, cancellations, and service trends.

• Maintain organized records and accurate reporting related to transport coordination activities.

• Utilize dispatch software, communication systems, spreadsheets, and reporting tools effectively.

• Escalate unresolved issues appropriately to leadership.

Minimum Qualifications

• High school diploma or equivalent required.

• Minimum of 2 years of customer service, healthcare coordination, dispatch, transportation, or account management experience.

• Strong communication and interpersonal skills.

• Demonstrated ability to work independently and manage competing priorities.

• Strong problem-solving and conflict-resolution skills.

• Ability to remain calm and professional in fast-paced healthcare environments.

• Proficiency with Microsoft Office, email systems, and computer-based scheduling platforms.

• Ability to effectively communicate with individuals from diverse backgrounds and professional levels.

Preferred Qualifications

• Experience working in hospitals, SNFs, rehabilitation centers, or healthcare transportation.

• EMS, NEMT, patient access, transfer center, or dispatch experience preferred.

• Experience with CAD systems, scheduling software, or transportation coordination platforms.

• Knowledge of insurance authorization and transport coordination processes is a plus.
