Location: LSC School Buildings
Hours: School Hours, up to 29 hours per week
Days: Monday through Friday as needed and available
Rate: LPN $21.58 per hour/RN $24.83 per hour

Substitute RN/LPN JOB DESCRIPTION
THE SUBSTITUTE RN/LPN ASSIGNED TO WORK IN LSC BUILDINGS WILL:





  1. Be licensed by the state of Indiana for practical nursing.
  1. Be CPR certified.
  1. Function under the supervision of the LSC nursing coordinator.
  1. Provide nursing/illness assessment and nursing treatments, and complete health charting on all students sent to the health office.
  1. Administer general first aid to injured students in accordance with the Lafayette School Corporation policies.
  1. Complete accident/incident reports when needed in accordance with LSC policy. 
  1. Contact parent as needed when student illness or injury to offer nursing advice or provide referral to health provider.
  1. Collaborate with school staff and parents regarding student health concern to promote highest possible student success while in the school environment.
  1.  Use LSC nursing calendar of duties to complete yearly tasks in a timely manner.
  1.  Assist with creating list of health concerns from health histories, communicates this list to pertinent teachers, principal, and director of health services.
  1.  Determine with building nurse the need for specific health care plan to be created by RN or Director of Health Services.
  1.  Function under the Lafayette School Corporation Physician's Standing Orders when providing care to students.
  1. Perform daily health procedures as directed by physician order or supervising nurse.  (i.e. asthma treatments, diabetic testing, catheterization, g-tube feeding etc.)
  1.  Administer medications according to corporation policy.
  1. Conduct vision screenings, make appropriate referrals, follow up care and chart results.
  1. Screen for head lice as needed and follow up with a classroom inspection if infestation of lice is evident.
  1. Assist with verifying that immunization records are current with state requirements and on file with the school.  Attaches or records immunization records in the students' PowerSchool health record.  Verifies immunization records are in the CHIRP state registry site and revises with new data as need.
  1.  Is a medical resource for teachers with student health concerns.
  1.  Attends scheduled nurse meetings.
  1.  Maintains health office records.
  1. Carry out other duties as outlined by Director of Health Services and the building principal.
