Job description

Job Summary and Qualifications

The Lead Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. 

Other Duties & Responsibilities:

  • Acts as a liaison between the patient and community resources/post-acute care providers.
  • Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. 
  • Facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. 
  • Provides orientation and mentorship to new and existing staff, collaborates with the Director of Case Management in the hiring and interview process for new professional staff and manages a limited case load in addition to providing guidance and consultation to social work staff. 

What qualifications you will need:

  • A Master’s Degree in Social Work is required
  • Licensure as a LMSW or a LCSW is required
  • At least 2 years of experience as a Social Worker is required
  • Prior experience in the acute care setting strongly preferred
