Job Overview:

RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.

Shift:

  • Full Time Days
  • Monday-Friday
  • Regional Onsite Coverage and Telemedicine Coverage

Qualifications:

  • All Applicants
    • CDS and DEA
    • BLS
    • Master's degree required or Graduate Level Certification
  • APNs
    • Current NJ APN
    • Nurse Practitioner Certification
    • RN License
  • PAs
    • PA license
    • NCCPA

Essential Functions:

  • Performs comprehensive initial and episodic health assessments; in order to identify existing or potential health problems
  • Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team
  • Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
  • Provides follow-up care including acute and routine examinations
  • Assesses progress, identify problems, initiate preventive measures, recognize emergent problems, initiate proper care in collaboration with physician
  • Monitors and adjusts patients medication according to protocol or medical plan of care
  • Implements nutritional therapies, as appropriate
  • Triages patient problem calls which requires medical attention
  • Orders appropriate laboratory and diagnostic procedures
  • Recognizes normal and abnormal laboratory findings and the pathology involved
  • Documents information in patients charts following documentation standard
  • Provides health education to the patient and family (e,g,, management of side effects, anticipatory teaching)
  • Manages medical emergencies as indicated, and according to standing orders
  • Provides monitoring and continuity of care between health care settings
  • Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues
  • Initiates proper referrals to private or community agencies
  • Acts as a resource and teacher for other members of the health care team
  • Shares knowledge and expertise with staff
  • Collaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of care
  • Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated

Travel Requirement:

  • Employee will be expected to provide both on-site and telemedicine support and coverage for practices as needed
  • Provides coverage for all practice sites within the assigned regional territory
  • Mileage reimbursement will be provided for travel to assigned locations from primary department
  • Frequency of travel will be contingent on the practice need for the assigned location
  • Duration of coverage will be contingent on the practice need for the assigned location
