• Deliver direct therapeutic services to clients dealing with major mental illnesses or severe emotional disorders, either virtually or as needed in community settings.
• Work cohesively as a part of a multidisciplinary team to achieve shared objectives, facilitating collaboration among healthcare providers, clients, and support staff.
• Respectfully interact with individuals from diverse backgrounds, adapting approaches to fit their unique cultural norms and values.
• Adhere to the organization’s ethical guidelines and code of conduct while participating actively in the financial structures and strategic planning of our services.
• Conduct thorough assessments, utilizing evidence-based screening tools and developing integrated care plans tailored to individual client needs.
• Provide ongoing support through crisis interventions, individual and group therapy sessions, ensuring that clients have access to essential support services.
• Maintain detailed documentation of client interactions, treatment plans, and progress reports, ensuring timely submission to facilitate coordinated care.
• Monitor and help facilitate admissions, treatment reviews, and discharges, ensuring clients receive appropriate care throughout their journey.
• Participate in committees aimed at enhancing service delivery and overall agency performance, contributing to ongoing quality improvement initiatives.

The clients served at Heal Street Wellness Center are largely from underprivileged backgrounds, many facing socioeconomic barriers that affect their mental health. Our LCSWs will work with diverse groups, including children, families, and adults, often encountering a range of issues from trauma and anxiety to chronic mental health conditions. This role is crucial in linking clients to necessary resources to ensure comprehensive treatment and support.

• A Master’s degree in Social Work (MSW) from an accredited institution is mandatory.
• Current Licensure as a Licensed Clinical Social Worker (LCSW) in Connecticut is required.
• Minimum of 2 years of clinical experience preferred, demonstrating proficiency in assessment, diagnosis, and treatment of mental health disorders.
• Strong verbal and written communication skills essential for effective case management and collaboration.
• Organizational skills and the ability to prioritize tasks effectively, ensuring high standards of care.
• Knowledge and competency in handling age-related and specialized issues pertinent to the populations served.
• Bilingual skills, particularly in English and Spanish, are preferred to better serve our diverse community.

• A background working in community health settings or with populations facing health disparities is desirable.
• Familiarity with electronic health record systems (EHR) and telehealth technologies is advantageous.
• Experience in crisis intervention and trauma-informed care approaches will enhance service delivery.
