Heal California Wellness Services is seeking a dedicated and experienced Senior Licensed Clinical Social Worker (LCSW) to join our compassionate team. This full-time, remote position allows you to deliver essential social work services to our diverse community in Fontana, California. Key Responsibilities
  • Document and maintain accurate and comprehensive patient records, including assessments, treatment plans, and case notes, in accordance with ethical guidelines and practice standards.
  • Conduct thorough psychosocial assessments to identify the medical, social, and emotional needs of patients and their families, using effective interviewing techniques and collaboration with healthcare team members.
  • Develop and periodically refine individualized treatment plans based on the ongoing assessments of patient concerns, strengths, and needs, addressing their unique spiritual and cultural values.
  • Utilize evidence-based practices, including behavior modification and supportive therapies, to support patients and families in navigating their challenges and achieving their goals.
  • Serve as a liaison with community resources, coordinating services that facilitate access to support systems and enhance patient care continuity.
  • Maintain proficiency in the current DSM diagnostic criteria to accurately identify and diagnose mental health disorders in children and adolescents, fostering resilience and growth.

Qualifications
