Dietician - PRN
What You Must Have:
  • Bachelor's Degree in nutrition or related subjects, preferred
  • Current State Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
  • Two years of experience as a Dietician in a clinical care setting, hospice or home health preferred.
  • Ability to establish and maintain effective working relationships with all segments of the staff, the lay and professional public, the Board of Directors and Medical Director.
  • Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible

What You Will Do
  • Provide direct dietary assessment of patient
  • Ability to establish plan of care to meet the dietary needs of the patient
  • Provide education to patient and caregiver regarding diet management
  • Appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
  • Provide counseling and education to IDG, nursing staff and physician, as appropriate
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
