Overview
As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.
Requirements
  • Graduate of an accredited Licensed Practical Nurse educational program
  • Current Licensed Practical Nurse / LPN license within the state of practice
  • Minimum of one (1) year of Licensed Practical Nurse experience in home health, hospice, hospital or skilled nursing facility; previous home healthcare experience preferred
  • Current CPR certification
  • Reliable transportation, valid driver's license and current automobile insurance
