Klarity Health Consultation and Counseling Center is seeking a dedicated and licensed Mental Health Counselor to join our passionate team in delivering impactful mental health care. You will work in a remote setting while also attending occasional meetings at our outpatient office located in San Jose, California.

• Conduct comprehensive mental health assessments that accurately reflect the patient's psychological state while considering any co-existing medical conditions.
• Design and implement individualized treatment plans that are integrative, addressing both mental health and medical issues, employing evidence-based therapeutic techniques.
• Facilitate individual and group therapy sessions that are tailored to each patient’s circumstances to maximize their engagement and promote personal growth.
• Collaborate with our medical team regularly to ensure continuity of care and effective treatment success, which may involve sharing insights from therapy sessions to enhance overall patient outcomes.
• Provide crisis intervention and immediate support to patients facing acute mental health challenges, ensuring their safety and guiding them through their crises.
• Educate patients about the intricate relationship between mental health and physical wellness, empowering them with knowledge to better manage their conditions.
• Maintain meticulous records and comply with legal and ethical standards of confidentiality and professionalism in all documentation.
• Participate actively in interdisciplinary meetings and case discussions to share progress on patient treatment and adapt strategies as needed.
• Stay informed of current best practices and research in integrated mental health care to continually refine your practice methodologies.

You will primarily work with individuals who face unique challenges due to co-occurring medical conditions that affect their mental health. These may include patients dealing with chronic illnesses, disabilities, and varying levels of socio-economic stress. Your role will be critical in helping them navigate these difficulties, fostering resilience and therapeutic rapport.

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.
• Evidence of expertise in addressing the mental health needs of clients who have concurrent medical conditions.
• Excellent interpersonal skills, fostering trust and rapport with patients.
• A commitment to maintaining professional boundaries and a deep understanding of ethical practices in mental health care.

• Familiarity with electronic health record systems and software used for treatment planning and documentation.
• Demonstrated ability to work extensively and effectively within a team to foster an interdisciplinary approach to patient care.
• Prior experience in crisis intervention and managing emergency mental health situations.

The salary for this position ranges from USD 115,000 to 130,000 annually, reflecting your level of experience and qualifications. Additionally, we offer a generous sign-on bonus of $3,000 to welcome you to our team.
