Job description

Job Brief:  Emergency Care Management (ECM) Care Coordinator

The Emergency Care Management (ECM) Care Coordinator provides support to the ECM Hub in transferring Kaiser members from non-preferred hospitals and Kaiser medical offices, Advanced Care Centers (ACC), and Clinical Decision Units (CDU) to our core and preferred hospitals. Responsibilities will include administrative duties to support repatriation, admissions, care coordination and transfer processes in partnership with the non-core QRM department

Our focus is on long-term health, effective prevention, diagnosis and treatment of disease.  Care is delivered across 40+ specialties at 26 medical offices featuring state-of-the-art equipment, labs, imaging services, and pharmacies. We also provide surgical services and around-the-clock care at some of the area's top hospitals.  We invite applications for an intern in Clinical & Health Systems based at our regional office.

What You Can Expect:

       Answer incoming calls to the ECM Hub via a queue line, retrieving voicemail messages and route calls appropriately.

       Obtain clinical information per guidelines.

       Opens appropriate ECM Hub Encounter in KPHC for each call received and documents accurate information.

       Verify benefits via KPHC and/or Member Services for members needing admissions, transfers, or repatriations.

       Screen KPHC in-basket Referral Request folder for members in noncore facilities, notifies RN, open encounter as needed, and document accurate information.

       Schedule follow-up appointments as requested.

       Outreach calls to hospitals to obtain rounding physician contact information as requested.

       Coordinate ambulance transport as per workflow.

       Perform other duties as assigned to include but not limited to:

o   Arrange outpatient blood transfusions/ blood patches/PICC lines appointments.

o   Other administrative duties in partnership with non-core QRM

       Knowledge of and compliance with all department policies and procedures

       Obtaining vital signs per guidelines.

       Participate in clinical discussion with MD or Hub nurse.

       Assist with training of new hires

       Provide ongoing support to new hires during their onboarding period.

Minimum Requirements:

       Graduate of an accredited Advanced EMT or Paramedic training program

       Medical Assistant training (certified MA, preferred)

       3 years of experience working in the healthcare field as a Medical Assistant or Paramedic or have previous experience/training in the ECM Hub to perform the above-mentioned responsibilities/functions.

       Excellent communication skills, to include verbal, written, and telephone skills.

       Excellent planning and time management skills to include the ability to handle multiple tasks.

       Ability to build effective working relationships with coworkers, hospitals, physicians, ambulance vendors, and other departments.

       Ability to diffuse problems that arise and promote a patient centered environment

Preferred:

       5 years of experience in the health care field and a familiarity with medical terminology.

       Experience working with Microsoft Excel and Word
