Key Responsibilities

• Conduct thorough mental health assessments that take into account the interplay of medical conditions impacting mental health.
• Develop and implement individualized treatment plans that integrate psychiatric and medical care, ensuring all aspects of a patient’s health are considered.
• Facilitate both individual and group therapy sessions, utilizing evidence-based methodologies tailored to the specific needs and circumstances of each patient.
• Work collaboratively with medical providers to ensure coordinated and effective care, maximizing treatment outcomes.
• Provide crisis intervention and emotional support during mental health emergencies, demonstrating the ability to act promptly and compassionately.
• Educate patients about the connection between mental health and physical well-being, empowering them with strategies to manage their conditions effectively.
• Maintain accurate and confidential documentation in compliance with legal and ethical standards, ensuring the safety and privacy of all patients.
• Participate actively in team meetings, case conferences, and interdisciplinary care planning to foster collaboration and communication within the team.
• Engage with ongoing professional development, keeping abreast of the latest research, best practices, and advancements in integrated care.

Patient Population / Client Focus

This role serves a diverse group of patients, many of whom face complex challenges due to co-occurring medical conditions, such as autoimmune disorders, chronic pain, and substance use disorders. Qualifications

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.
• Demonstrated experience in addressing mental health needs for individuals with co-occurring medical conditions.
• Exceptional interpersonal and communication skills, allowing for effective interactions with patients, families, and healthcare professionals.
• Proficient in maintaining professional boundaries while adhering to ethical guidelines and ensuring patient confidentiality.
• Familiarity with various computer applications and electronic health record systems to manage documentation effectively.
• Proved ability to work collaboratively as part of an interdisciplinary team, fostering a spirit of cooperation and shared purpose.

Preferred Experience

• Experience working in a community health clinic or similar healthcare environment with diverse patient populations.
• Prior experience with integrated care models that emphasize the coordination between mental and physical health services.
• Familiarity with evidence-based therapies, such as Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT).

Compensation and Benefits

This full-time position offers a competitive salary in the range of USD 100,000 to 120,000 annually, alongside a comprehensive healthcare benefits package.
