  $1,250 Hiring Bonus! 
At Adelante, you will play an important role in supporting our team and the individuals we serve. As our Service Coordinator, you will help implement, Coordinate and monitor client services and therapy plans as well as staff training. You will work closely with Adelante staff, case managers, guardians and families of clients to ensure that individual service plans are meeting the needs of each client; and act as a liaison and advocate for client services.


What You'll Do:


The following statements are intended to describe the general nature and level of work being performed. They should not be construed, as an exhaustive list of all responsibilities, duties and skills required of personnel so classified. 


  •  Provide direct client support while working with clients; Hands on as needed.
  • Able to implement goals, objectives and program plans for all assigned clients
  • Provides Crisis management, monitor therapy programs, monitors client individual support plans (ISP's) 
  • Coordinates the individualized service planning system to maximize each consumer's progress;
  • assists in implementing therapy plans; ensures communication between internal and external agency providers is clear and addresses the needs of the client
  • facilitates client meetings and staff in-services as needed. 
  •  carries out the ISP meeting process; coordinates inter and intra-agency services; writes and implements individual training plans; trains consumers.  
  •  implements all aspects of CaseLink ISP managements software; ensures data collection on individual goals is performed regularly and accurately
  •   completes all required reporting on consumer service plans, programs and progress in a timely manner; provides required reports to Medicaid Waiver and other agencies as needed.  
  • Responsible for monitoring client funding; assures adequate and current funding is in place for all clients on case load
  • Knowledgeable of contracts and performance standards as related to position
  • Serves as a resource person to each work team; identifies program strengths and weaknesses and assists in developing programs to maximize effectiveness. 
  • May assist in the development of the Resource Center and the physical layout of the program including but not limited to: development of resource materials list and submit orders to the Director of Employment Services; identify web sites appropriate for the consumers to access that will assist them in the job development process; monitor the use of the Resource Center and all materials assigned to the Center; develop employment related training curriculums; teach employment related classes to groups of participants referred from Adelante programs and any other outside resources. 

Schedule & Pay:


Job Type: Full Time|  Monday thru Friday  8:00am - 4:30pm


Pay from: $17.50 per hour

Requirements

  • High school diploma or GED required.
  • Knowledge of therapies (PT, OT, SLP, etc.) preferred but not a must 
  • Comfortable speaking and communicating in meeting environment
