Our Candidate


We are seeking compassionate and motivated Licensed Independent Social Worker to perform consultations in various environments including nursing homes and assisted living facilities. OnSite providers can build an autonomous practice while making a positive impact on the lives of older adults and their families. As a Licensed Independent Social Worker working at OnSite you will be provided full administrative support and clinical mentorship with a team approach. Reponsibilities & Duties

  • Conduct comprehensive client assessments to identify mental health needs
  • Develop and implement personalized treatment plans
  • Provide individual and group therapy sessions
  • Offer crisis intervention and support to clients in distress
  • Collaborate with other healthcare professionals to coordinate client care
  • Advocate for client resources and services within the community
  • Maintain accurate and confidential client records
  • Evaluate treatment outcomes and adjust plans as necessary
  • Educate clients and their families about mental health issues
  • Participate in continuing education to stay current with best practices


Requirements


  • Master's degree in social work from an accredited institution (MSW)
  • Clinical licensure (LISW or equivalent) in the state of practice
  • Specialized training in specific therapeutic modalities (e.g., CBT, DBT)
  • Experience working with diverse populations
  • Knowledge of community resources and services
  • Strong crisis management skills
  • Excellent written and verbal communication skills
  • Proficiency in electronic health record (EHR) systems
  • Minimum of 1 year of post-graduate clinical experience preferred
