Job description

Overview
Helio Health is looking for a Peer Case Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in our low barrier OMH homeless scattered site supportive housing units. This case manager would support individuals in maintaining housing, navigating health and human services as well as skill development. Provide in-community, in-person support at least 4 times per month upon entry to the program. PCM will work with the individual, in collaboration with collateral supports and services to develop and execute an ISP, with goals and objectives that focus on housing retention, community integration and recovery, reviewing and updating the plan on a quarterly basis. This is aFull-Time position and located at our Supportive Housing First Rochester Program.
Pay Rate: $20.00- $24.00 per hour.
In addition to Helio Health's comprehensive benefits package, Full-Time hires for this position are eligible for a Sign-On Incentive up to $1,500!
Responsibilities
  • Maintains a case load and performs primary case work responsibilities including:
    • Intake, orientation
    • Referrals
    • Counseling
    • Crisis intervention
    • Patient/resident education
    • Case management
    • Reporting and record keeping
    • Consultation with other professionals
    • Coordination of care with other community providers
    • Discharge planning
  • Performs Quality Assurance activities
    • Provides services according to agency policies, procedures, program guidelines, and governmental regulations.
    • Monitors provision of services when assigned.
    • Evaluates programs and identifies strengths/weaknesses.
    • Assists in problem resolution and implementation of supervisor approved program modifications.
  • Participates in program administrative and clinical committees as assigned.
  • Represents agency to the community.
  • Other duties as assigned by supervisor.

Qualifications
  • High school diploma required, associate's degree in a related human services field preferred.
  • Individuals with lived experience preferred.
  • Lived experience with homelessness, behavioral health and/or substance abuse issues.
  • Valid NYS Driver's License preferred.
