Home Infusion Pharmacist – Sterile Compounding Position Overview
We are seeking a licensed Pharmacist with strong home infusion and sterile compounding experience to support a high-acuity pharmacy environment. This role is focused on clinical oversight, sterile preparation standards, documentation accuracy, and final verification responsibilities rather than prescription data entry or routine dispensing workflow.
Candidates must have prior experience working in a clean room environment and performing or overseeing sterile compounding procedures.
Home Infusion Pharmacist – Sterile Compounding Key Responsibilities
- Perform final prescription verification and clinical checks
- Review laboratory values and patient-specific clinical documentation
- Ensure accuracy, safety, and compliance of sterile compounded preparations
- Compound sterile medications following USP and aseptic technique standards
- Monitor patient medication therapy, compliance, and potential overuse patterns
- Evaluate prescription suitability and ensure regulatory compliance
- Maintain complete and accurate patient medication records and pharmacy documentation
- Provide clinical consultation on medications including:
- Dosage and administration
- Side effects and drug interactions
- Storage and handling requirements
- Educate patients on medical devices (e.g., blood pressure monitors, glucose meters)
- Support clinical programs including smoking cessation, blood pressure monitoring, and cholesterol management
- Identify and resolve drug-related problems in collaboration with physicians and healthcare providers
- Communicate effectively with physicians, nurses, and pharmacy staff
- Supervise pharmacy technicians, clerks, and interns as needed
- Maintain inventory of pharmaceutical and medical supplies
Home Infusion Pharmacist – Sterile Compounding Required Qualifications
- Doctor of Pharmacy (PharmD) degree required
- Active state pharmacist licensure required
- Prior home infusion pharmacy experience required
- Strong experience in sterile compounding / clean room operations required
- Knowledge of USP standards and aseptic technique
- Strong clinical judgment and patient safety focus
- Ability to work independently in a fast-paced clinical environment
- Strong communication and documentation skills
