Cantata Adult Life Services, formerly known as The British Home, is a nonprofit, mission-driven community with over 100 years of experience, offering a full continuum of care on a beautiful 10-acre campus in Brookfield, Illinois. We are seeking strong, compassionate leaders to join our team and help us continue enriching the lives of older adults through exceptional care and a vibrant community.


The primary purpose of your job position is to efficiently deliver care to residents utilizing the assessment process (i.e., assessment, planning, intervention, implementation, evaluation). Responsibilities and care will be directed by the Woodlands Assisted Living Manager with clinical oversight by the Administrator; and must be in accordance with department and Cantata’s policies and procedures, current federal, state and facility standards, guidelines and regulations governing our company.


Principal Duties and Responsibilities: This description is intended only to identify the basic responsibilities and requirements of this job classification and does not prevent the assignment of other job-related duties. Successful job applicants will be able to perform these essential functions with or without requested reasonable accommodation.

  1. Performs all duties in accordance with the Cantata Promise, treating every person (Client, fellow employee-partner, general community member) with respect, integrity and compassion.
  2. Ensures resident safety and well-being through observation and assessment of resident and communication of resident needs and changes among clinical team, resident and/or responsible party.
  3. Completes medication ordering, set up, supervises, monitors medication administration, and administers medication and treatment, as scheduled according to the service plan.
  4. Maintains accuracy of resident service plan with frequent review and timely updating of observed and necessary changes.
  5. Holds RA/CNA staff accountable to delivering resident care according to resident service plan and needs.
  6. Provides counsel and education to the resident and family support system, as necessary. Encourages independence through wellness and exercise programs.
  7. Responsible for notifying resident family member(s) in cases of illness/emergency/hospitalization. Ensures that the Woodlands Manager is properly informed.
  8. Ensures that all required documentation and communication is completed in the time frame(s) determined along with the appropriate follow up steps that are needed, including required and relevant documentation in the electronic medical record (EMR).
  9. Responsible for timely and effective communication to the Woodlands Manager, Resident Assistants as well as to other department managers and staff to ensure that the needs and care of the resident(s) are being met.
  10. Coordinates shift-to-shift responsibilities ensuring that the communication log, pagers, spectralink phone system as well as any emergency coverage/systems are operational utilizing procedures and forms established by Cantata for this purpose.
  11. Assures that sufficient supplies and equipment are available for provision of resident care.
  12. Makes written and oral reports as required to physicians, pharmacists, therapists, and other providers of service to enable proper follow-through of orders, coordination of care services, communication of nursing observations suggesting the need for further medical evaluation. Notifies residents or person(s) responsible for the resident of such orders or information to ensure resident and/or responsible party is informed of any changes in orders or condition..
  13. Acts on behalf of the Woodlands Assistant Living Manger in his/her absence.
  14. Consistently demonstrates an ability to respond to changing situations in a flexible manner
  15. Attends in-services, meetings and educational training in accordance with Cantata’s policies and procedures.
  16. Performs tasks/duties and utilize forms and systems in accordance with department and Cantata’s policies and procedures.

B. Contact with Others

Extensive contact with residents on a daily basis. Daily contact with all levels of personnel within the company. Some interaction with resident’s family members and community contacts.


C. Delegation of Authority

As Wellness Nurse you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.


D. Responsibility for Accuracy

Omission or incorrect mandatory federal, state and company documentation can generate penalties enforced by regulators.


E. Responsibility for Confidential Data

Employees are required to safeguard sensitive information. Sensitive information includes, but shall not be limited to, information concerning Cantata’s former and current residents/clients, medical and health information and Cantata’s information systems including but not limited to the Internet and E-mail.

F. Physical Requirements/Working Conditions

  1. Must meet the general health requirements set forth by the policies of this facility.
  2. Must be able to push, pull, move, and/or lift a minimum of twenty-five (25) pounds.
  3. Is subject to callback during emergency conditions and may be necessary to assist in the evacuation of residents.
  4. Individual will be scheduled based on operational need.
  5. Works in residents’ rooms and areas designated for resident care.
  6. Works beyond normal working hours and on weekends and holidays when necessary.
  7. Sits, stands, bends, lifts, walks and climbs stairs, as needed to conduct nursing activities.

G. Exposure Classification

  1. Job duties are classified as Category I (routine exposure to blood, body fluids or tissues).
  2. Routine exposure to chemicals which are identified for use in the nursing department.


Requirements

  1. Current valid certification as a Registered Nurse or Licensed Practical Nurse in the State of Illinois.
  2. Current CPR certification.
  3. Must possess leadership ability and willingness to work harmoniously with and to direct other personnel.
  4. Ability to communicate effectively both verbally and in written form with a diverse group of people.
  5. Ability and willingness to effectively motivate and lead individuals and groups with diverse needs and abilities.
  6. Ability and willingness to work harmoniously within a team environment departmentally.
  7. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  8. Must possess the ability to make independent decisions when circumstances warrant such action.
  9. Must be able to maintain good personnel relations and employee moral.
  10. Aptitude for detail work.
  11. Basic computer literacy and familiarity with EMR or willingness to learn EMR system.
  12. Must be able to read, write, speak, and understand the English language.
  13. Must be able to cope with the mental and emotional stress of the position.
  14. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
