• Conduct thorough psychosocial assessments of patients and families, identifying their social, emotional, and physical needs upon admission and at required intervals, utilizing established social work frameworks.
• Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive psychosocial treatment plans, ensuring seamless coordination of referrals to specialty care, social services, and broader community resources.
• Provide meaningful support to patients and families grappling with the emotional and psychological ramifications of illness, focusing on coping strategies and adaptive techniques.
• Manage a diverse caseload with varying levels of clinical acuity, balancing the immediate needs of patients while promoting long-term strategies for mental wellness.
• Participate actively in case identification efforts for patients with potential psychosocial concerns, contributing to team conferences and interdisciplinary rounds to advocate for their needs.
• Deliver supportive counseling, grief therapy, and crisis intervention services, empowering individuals and families as they navigate complex healthcare situations and transitions.
• Engage patients and families in psychoeducation regarding diagnosis, treatment options, and prognosis, facilitating smoother transitions from youth services to adult care as necessary.
• Contribute to quality assurance initiatives and community improvement projects related to mental health services, including assisting in the development of relevant policies and guidelines.
• Provide clinical supervision and mentorship to MSWs and student interns, fostering the next generation of mental health professionals.
• Maintain accurate and comprehensive electronic medical records, ensuring compliance with organizational standards and facilitating effective communication between team members.

As a Licensed Mental Health Counselor at Harbor of Hope, you will serve a diverse patient population that includes adults, seniors, adolescents, and children. This diverse clientele often presents a variety of mental health challenges, from adjustment disorders to severe emotional and behavioral issues. Your role will be essential in advocating for each patient's unique needs, ensuring they receive tailored support and resources to thrive.

• A valid California license as an LCSW, LMFT, or LPCC is mandatory.
• A master's degree in a relevant behavioral science field from an accredited institution is required.
• Strong competency in writing detailed case narratives and clinical reports, demonstrating effective assessment, interviewing, and counseling techniques.
• Proficient knowledge of social work theories, principles, and methodologies relevant to diverse client needs.

• Experience in a community health setting or in roles that require collaboration with multidisciplinary teams is advantageous.
• Previous engagement in training or supervising interns or junior staff can enhance your candidacy.
• A background in crisis intervention and grief counseling will be highly beneficial.

This full-time position offers a competitive salary range of USD 120,000 to 130,000 annually, commensurate with experience.
