Key Responsibilities
  • Conduct thorough psychosocial assessments of patients and families, identifying their social, emotional, and physical needs upon admission and at required intervals, utilizing established social work frameworks.
  • Collaborate with interdisciplinary healthcare teams to develop and implement comprehensive psychosocial treatment plans, ensuring seamless coordination of referrals to specialty care, social services, and broader community resources.
  • Provide meaningful support to patients and families grappling with the emotional and psychological ramifications of illness, focusing on coping strategies and adaptive techniques.
  • Manage a diverse caseload with varying levels of clinical acuity, balancing the immediate needs of patients while promoting long-term strategies for mental wellness.
  • Participate actively in case identification efforts for patients with potential psychosocial concerns, contributing to team conferences and interdisciplinary rounds to advocate for their needs.
  • Deliver supportive counseling, grief therapy, and crisis intervention services, empowering individuals and families as they navigate complex healthcare situations and transitions.

Patient Population / Client Focus

As a Licensed Mental Health Counselor at Harbor of Hope, you will serve a diverse patient population that includes adults, seniors, adolescents, and children. Qualifications
