LPN (Licensed Practical Nurse)

As a LPN (Licensed Practical Nurse), you will play a vital role in providing high-quality patient care and support to our residents. About the Role:

  • As an LPN, you will work closely with our interdisciplinary team to deliver individualized care to our patients, focusing on their unique needs and preferences.

  • You will administer medications, perform vital signs, and provide basic wound care, among other responsibilities.

  • You will also document patient information, maintain accurate records, and communicate effectively with patients, families, and healthcare providers.

Responsibilities:

  • Provide direct patient care, including medication administration, vital signs, and basic wound care.

  • Document patient information and maintain accurate records.

  • Participate in interdisciplinary team meetings to discuss patient care and concerns.

  • Assist with activities of daily living (ADLs) and maintain a safe and clean environment for patients.

Requirements:

  • Licensure: Current LPN license in the State of Wisconsin.

  • Education: LPN diploma or degree from an accredited nursing program.

  • Experience: Previous LPN experience in a long-term care setting preferred, but not required.
