Clinical Pharmacist, Solid Organ Transplant | Boston Childrens Hospital

Position Summary: The Pediatric Cardiac and Pulmonary Transplant Clinical Pharmacist will serve as an integral member of a busy, multidisciplinary heart and lung transplant program at Boston Children's Hospital, providing comprehensive pharmacy services throughout the transplant continuum. Responsibilities will include direct clinical care of pediatric heart and lung transplant patients, medication selection and optimization, therapeutic drug monitoring, drug interaction management, patient and family education, medication reconciliation, and collaboration with physicians, nurses, and other members of the transplant team. The pharmacist will also play a key role in ensuring compliance with CMS and other applicable transplant regulatory and quality requirements, including maintaining appropriate documentation, monitoring program metrics, and supporting ongoing readiness for regulatory review.

Key Responsibilities:

  • Completion of a chart/med review for all heart/lung transplant patients daily
  • Attendance at both Cardiac and Pulmonary multidisciplinary transplant rounds at a minimum of 2 days per week
  • Participation in transplant patient selection team meetings
  • Participation in transplant evaluations - The SOT Pharmacist meets with patients/families for evaluation and will document in medical record
  • Documentation of Admission Notes in EMR for patients coming in for transplant surgery with communication of plans with the clinical team
  • Pharmacy Consultation and Drug Information as SME - For ALL SOT Teams & Transplant Nursing Staff SOT Pharmacist to be available on weekdays
  • Interpretation and evaluation of medication orders to ensure drug and dosage are appropriate for patient and to identify possible adverse reactions, contacting physician as needed.
  • Optimization of immunosuppressive regimens, including induction, maintenance, and treatment of rejection
  • Completion of medication history documentation.
  • Participation in quality improvement activities.
  • Completion of Discharge Education - Complete medication teaching with patient/family and document in EMR

Minimum Qualifications:

  • Knowledge of theories, principles, and concepts typically acquired through completion of a Bachelor's degree in Pharmacy plus up to one year of post‑degree course work and/or an internship and: two to three years general pharmacy experience, or one year directly related pediatric experience. Doctorate degree in Pharmacy preferred.
  • Current license and registration by the Massachusetts Board of Registration in Pharmacy.
  • Analytical skills to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems OR design relatively complex systems and programs.
  • Communication skills in order to effectively deal with conflicting views or issues and mediate fair solutions and well‑developed writing skills.

Schedule is days and every 4th weekend
