FUNCTION: To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required.

Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures.

Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements.

Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities. Attend and/or facilitate Family program when requested by supervisor. Participates in facility program and quality improvement activities.

Participates in quality improvement activities related to position and as requested by program director.

Responds to emergency situations in the facility by following appropriate procedures for ensuring patient safety during emergency procedures. Supervises interns and volunteers.

Is actively involved in milieu management through attendance at community meetings, supervision of patients during activities and interventions with patients found in violation of program/ facility rules. Participates in Performance Improvement Initiatives.

Maintains professional competence in discipline and treatment specialty areas through continuing education and supervision.

High school diploma, general education degree (GED) or bachelor’s degree with CASAC certification or LMSW, LCSW, LMHC. Background checks, pre-employment & drug screenings required. We are an equal opportunity employer according to all current standards.
