Job Summary

We are seeking a Family Nurse Practitioner (FNP) to join our growing family medicine practice. This role will focus on the management of diabetes, hyperlipidemia, obesity, and other common metabolic/endocrine conditions within a primary care setting. The Nurse Practitioner will work collaboratively with physicians and the healthcare team to provide comprehensive, patient-centered care across the lifespan.

This position is ideal for an NP with strong interest and experience in chronic disease management, preventive care, and patient education.

Key Responsibilities

Clinical Care

Diagnose, treat, and manage Type 1 and Type 2 diabetes

Manage hyperlipidemia, metabolic syndrome, obesity, and insulin resistance

Monitor and adjust medications including:

Oral hypoglycemics

Insulin regimens

GLP-1 agonists

Lipid-lowering therapies

Interpret laboratory results (A1C, lipid panels, CMP, TSH, etc.)

Provide preventive care and routine primary care services

Perform comprehensive and focused physical exams

Identify patients requiring referral to endocrinology or other specialists

Patient Education & Counseling

Educate patients on:

Diabetes self-management

Nutrition and lifestyle modification

Medication adherence

Weight management

Provide counseling on cardiovascular risk reduction

Support shared decision-making and goal setting

Care Coordination & Quality

Participate in quality improvement initiatives (HEDIS, value-based care)

Ensure compliance with evidence-based clinical guidelines

Document accurately and timely in the electronic medical record (EMR)

Qualifications

Required

Masterโ€™s or Doctorate degree from an accredited Nurse Practitioner program

Board certification as a Family Nurse Practitioner (FNP)

Active state NP license and DEA (or eligibility)

Eligible for credentialing with insurance payers

Strong clinical skills in primary care and chronic disease management

Preferred

Experience managing diabetes and lipid disorders

Familiarity with insulin management and newer diabetes medications

Interest in metabolic health and preventive cardiology

Experience in outpatient primary care or internal medicine

Work Environment & Schedule

Outpatient family medicine setting
