Key Responsibilities
  • Deliver skilled nursing care to patients in their homes under the supervision of an RN
  • Administer medications, treatments, and wound care as prescribed
  • Monitor patient conditions and report changes promptly to the RN and care team
  • Educate patients and caregivers on care plans, medications, and disease management
  • Accurately document all patient interactions in compliance with regulatory standards
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Support patients in achieving optimal health outcomes and independence

Qualifications
  • Current, unencumbered LPN license in the state of Oregon
  • Minimum 1 year of nursing experience (home health experience preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently in a home-based setting
  • Valid driver's license, reliable transportation, and current auto insurance
