Job description

Job Overview
Utilizing the nursing process, the Home Health Registered Nurse will manage and deliver comprehensive home health services, including assessments, interventions, and supportive care to clients primarily within their place of residence. Depending on the circumstances, duties may also include telephone triage, crisis intervention, problem solving, patient/caregiver advocacy and support, with emphasis of avoiding hospitalization in the off hours including weekends or holidays. As a key member of the health care team, this position must interact courteously and effectively with patients and their families as well as with co-workers from all Agency departments, community resources, and with patients' physicians to facilitate safe and efficient patient care.
Location: Lowell, MA
Hours: Part-Time, 16 hours, Days, Weekends Only!
Minimum Qualifications:
1. Massachusetts RN Licensure.
2. Valid state issued Driver's License.
3. One (1) year of acute medical/surgical nursing experience.
4. Cardiopulmonary Resuscitation (CPR) Certification.

Essential Functions:
  • Assess and implement physical and psychosocial needs of the patient following established Standards of Nursing Practice and VNA procedures.
  • Assess and implement patient/family learning needs, style and limitations and makes adjustments for delivery of information.
  • Identify abuse and/or neglect and identifies reporting mechanism per policy.
  • Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Demonstrates knowledge of the principles of growth and development over the life span.
  • Possesses communications skills needed to effectively understand patient's reactions or responses to treatment.
  • Involves the patient in understanding and become involved in the course of treatment.
  • Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • Develops a realistic written plan of care toward measurable goals.
  • Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregivers.
  • Provides ongoing interdisciplinary assessment of the patient.
  • Plans death at home when appropriate with patient/family and other members of the health care team.
  • Completes physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies.
  • Maintains and demonstrates a continuous awareness of the health needs and resources of the community.

Physical Requirements:
1. Frequently required to move about inside the office to access file cabinets, office machinery, etc.
2. Ability to remain in a stationary position 90% of the time.
3. Occasionally lifts and/or move up to 10-25 pounds.
4. Constantly operates a computer and other standard office equipment such as copy machine, phones, photocopier, fax machines and computer printers.
5. Specific vision abilities apply including close vision, distance vision and ability to adjust focus.
6. Noise level in the work environment is usually moderate.
