Job description

Position: Registered Nurse Home Health Nurse
 Salary: salary + Benefits
 Shift Timings: Monday-Friday
About the Position:
 1.  The goal of the home health Registered Nurse (RN) is to return the patient to the highest practicable level of independence and well-being within the community with self-management of disease and other identified processes.
 2.  When the highest practicable level of well-being is not independence, the goal is to prepare the patient and family/caregivers for a continuum of care within the home and/or within other community-based health care systems.
Qualifications:
 1.     A graduate of an accredited, approved school of professional nursing
 2.     Active license in the State of Colorado in good standing.
 3.     Must be able to communicate in the language of the client and document client related-activity as appropriate
 4.     Current driver’s license
 5.     At least one (1) year of experience as a Registered Nurse. Home Health experience is a plus.
 6.     Familiarity with OASIS preferred
