Responsibilities

Summary:
The Licensed Practical Nurse – Intake Coordinator initiates and provides comprehensive nursing care in a correctional setting while observing and maintaining the requirements of operational security.

Primary Duties and Responsibilities:

  • The Licensed Practical Nurse - Intake Coordinator works collaboratively with the intake staff and monitors the flow of intake to ensure patients are seen and processed by Health Services staff within the appropriate timeframe.
  • The Licensed Practical Nurse - Intake Coordinator assists with intake screenings and processing to ensure timely care is provided for the patients.
  • The Licensed Practical Nurse - Intake Coordinator provides routine reporting on intake processes to the Health Services leadership as part of the Quality Assurance/Quality Improvement process.
  • The Licensed Practical Nurse - Intake Coordinator assists in the performance of completing clinical screenings and collection of medical history as required on each patient entering the correctional system, if permitted by the client. Medical problems are referred to the physician/advanced practice
  • provider for extended evaluation.
  • Assists the physician with sick calls by screening patients and minor surgical procedures. Treats patients with problems within professional capabilities and refers other medical problems to the appropriate physician.
  • Ensures the implementation of provider’s orders. Ensures that prescribed medications are administered as ordered using the formulary. Applies knowledge of drugs and therapeutics in administering medication.
  • Makes observation rounds on patients, and records in progress notes. Evaluates symptoms, reactions, and progress of patients on a periodic basis, charts, and reports problems to management.
  • Maintains security while administering medications and is continually cognizant of the potential hazard of drug hoarding and trafficking among patients. Maintains non-involvement in the security aspects of care.
  • Destroys and properly disposes of all needles/syringes.
  • Prepares regularly scheduled reports and special reports, as necessary.
  • Maintains compliance with industry and company policies for accreditation. Coordinates intake-related laboratory duties, including specimen collection support, requisition completion, lab tracking, timely follow-up, and escalation of abnormal or critical results to the RN, provider, and Health
  • Services leadership as applicable.
  • Assists with intake-related diagnostic services and continuity of care, including EKGs, imaging, and other ordered diagnostic studies; ensuring results are obtained, routed for provider review, and documented in the health record.
  • Reviews intake records for pending laboratory or diagnostic orders and communicates outstanding results, referrals, or required follow-up to the appropriate clinical staff to support timely patient care.
  • Identifies urgent or emergent findings during intake screening or follow-up and immediately escalates concerns to the RN, provider, Health Services Administrator/designee, custody as appropriate, and emergency medical services/911 in accordance with facility protocol.
  • Maintains accurate and complete intake medical records by verifying required forms, screenings, consents, medication history, allergies, vital signs, provider orders, and follow-up documentation are completed and entered, scanned, or filed in the electronic health record as required.
  • Supports medical records duties by tracking incomplete intake documentation, communicating deficiencies, assisting with record reconciliation, and maintaining confidentiality of protected health information in accordance with company policy and applicable regulations maintaining confidentiality of medical records.
  • Performs other duties as assigned.

Qualifications

Minimum Requirements:

  • Graduate of an accredited school of nursing required. Must have a current, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state where the facility is located. Maintains a current copy of license on file at the facility at all times.
  • Two (2) years of direct work experience Licensed Practical Nurse preferred.
  • Must possess competence in the performance of the following services: crisis intervention, medical emergencies, clinical screening, medical nursing care, and psychiatric nursing care.
  • Must have the ability to work without direct, immediate supervision in performing nursing diagnosis, and be able provide appropriate follow through treatment.
  • Ability to work with computers and the necessary software typically used by the department
